Founded by Crystal Hayes, the community aims to offer personalized care and life enrichment through compassionate care. Transition Assisted Living provides a luxury living experience while being more affordable than other facilities in the area, offering a range of services including assisted living, memory care, and home care. The community is in the process of building an independent living community. Transition Assisted Living is located in Gladstone, Michigan, and offers a variety of living options including private rooms, shared rooms, couple's rooms, and hospice rooms, all designed with comfort and convenience in mind. The building features one floor with a gracious foyer, a conversation area, a coffee bar, and designated areas for dining and living. The facility has a total area of 6848 square feet, accommodating up to 20 residents with 17 bedrooms and 5 bathrooms. Safety and security are top priorities, with 24-hour video surveillance, secured access, and fully-sprinkled fire suppression systems throughout the building. The facility was newly renovated and inspected in 2022 to ensure compliance with modern codes and standards. Transition Assisted Living aims for higher staffing ratios than the state requires, ensuring personalized care for each resident.
