New Horizons Limestone, situated at 2020 Beverly Road in Gainesville, GA, and its sister facilities in Lanier Park and Habersham, are equipped with Transitional Care Centers (TCC). These centers focus on providing short-term rehabilitation to patients needing additional care to transition from hospital to home. With the goal of achieving this transition within 30 days, patients receive coordinated rehabilitation services from physical therapists, speech pathologists, and occupational therapists.
