Horizon Recuperative Care

With displaced patients in the LA/Orange County area experiencing hospital stays that are, on average, four days longer, the provision of recuperative healthcare becomes imperative to ensure they receive essential post-hospital supervision and treatment. Horizon offers a comprehensive level of follow-up healthcare for every patient, all at a fraction of the cost for both hospitals and insurance companies. Our services encompass monitoring and managing each patient's post-hospital medical recovery, including wound and injury care, daily vitals and medication management. We go the extra mile by facilitating transportation to and from appointments and assisting with prescription pickups. In addition to physical health, we prioritize mental well-being by conducting assessments and facilitating discussions with mental health professionals. We also make referrals for longer-term mental health care. Collaborating closely with individuals and their families, we formulate optimal plans to facilitate a successful transition back into the community. Our overarching goal is to significantly impact people's health, well-being, and quality of life by addressing Social Determinants of Health (SDOH). As a participant in the California Advancing and Innovating Medi-Cal (CalAIM) program, we are actively contributing to efforts aimed at enhancing health outcomes for all Californians. CalAIM, a state initiative, focuses on improving access to quality care, fostering better coordination among healthcare providers, and reducing overall costs.
