ElderONE reimagines senior care by providing all-inclusive medical, emotional, and social support, allowing seniors to live at home while maintaining their health and safety. As the region's only program of its kind, ElderONE offers personalized care through an expert, coordinated team, ensuring independence at home is possible for as long as possible. This innovative approach forms the complete care continuum for seniors 55 and older, allowing them to avoid long-term nursing home placement. ElderONE is a Program of All-Inclusive Care for the Elderly (PACE) that serves Monroe, Ontario, and Wayne counties, providing integrated health plans that promote independence and prevent hospitalizations. ElderONE operates several PACE Centers where the majority of care services are provided. These centers are located based on participant preferences and needs, and participants attend as determined by their individualized care plans. Locations operate from 8:30 AM to 5:00 PM, Monday through Friday. The program emphasizes an interdisciplinary approach involving geriatricians, nurses, social workers, therapists, and more, ensuring a holistic care plan is tailored and adjusted to the evolving needs of participants.
