BASIC SERVICES The basic services, described below, are based on the resident's pre-placement appraisal and are included in the monthly rate. ACCOMMODATION Every resident will have a private furnish bedroom with a private bathroom. You are encouraged to personalize Your room by providing your own furnishings and personal property, if not we will provide you with a bed, nightstand, dresser, rug, lamp, chair/recliner, TV. You are also entitled to use and enjoy with all the other residents the indoor and outdoor common areas. PHONE ACCESS Every resident has access to the facility telephone to make and receive calls. We require reimbursement for long distance calls.

BASIC SERVICES: (The basic services, described below, are based on the resident's pre-placement appraisal and are included in the monthly rate) Medication Management | We assist you with the administration and storage of your meds, stored separately from the other residents in a locked centralized location. We will also assist you in taking self-administered meds to the extent allowed by state law. We offer assistance in communicating with your physician and pharmacy for new/old prescriptions and refills. 24/7 Care & Supervision | Continuous care and supervision 24 hours a day, 7 days a week. Assistance with Activities of Daily Living | Activities of the daily living (ADL) such as, dressing, eating, toileting, bathing, grooming, transferring, mobility tasks and any other personal care needs requested, to the extent allowed by applicable state law. Repositioning the resident in bed to avoid bed sores, tray service and sponge bath in bed shall be available for minor temporary illnesses or recovery from surgery. Personal Hygiene | Personal Hygiene items of general use. These items are soap, toothpaste, shampoo, conditioner, toilet paper, gloves and wipes. We offer personal laundering service for all residents. Clean bed and bath linens are provided weekly, or as needed. Room and Bathroom will be cleaned every day or as needed. Appointments Management | Staff is available to assist with scheduling appointments for medical, dental or any other needs. Transportation | Will provide assistance with transportation arrangements. Incontinence Supplies (optional additional service)

MEALS & SNACKS Three nutritionally well-balanced meals and two snacks per day are included. Tray service will be provided in case of temporary need. Modified diets will be available, if prescribed by your physician as a medical necessity.
